Eligibility Requirements
Recipient is a U.S. citizen or otherwise authorized to work in the United States
• Recipient is accepted, and will be attending a university, college, or other post High School
educational institution in the fall of 2019
• Academic achievement as reflected by an Indiana High School cumulative minimum 3.0
GPA
• Applicant must have volunteered at an Indiana 501©(3) non-profit.
Application Process
A completed Scholarship Application Form
• One to three (1-3) page typed essay
• An official and complete copy of high school transcripts
• An acceptance letter from an accredited University or Trade School
• Proof of legal residency in the U.S. (i.e., birth certificate, passport, permanent resident card,
etc.)
DEADLINE: Completed applications must be submitted no later than May 31, 2019. Application
